Passport journey steps
Step 1: Documents we'll review for your application
With our Fast Track Passport Service, we'll review all your documents for accuracy and completeness before your passport agency appointment. Please send us:

Birth certificate of the baby - a scanned copy or a photo (for each baby)

Baby's height (in feet and inches), eye color, hair color (for each baby)
3
Copy of each parent's passport — photo of the main page (with photo and signature) and the cover (for each parent)
4
Your home country permanent address
5
Your email addresses (if different from the ones you use with SurroPlans)
6
Your US address (where the passport may be mailed — make sure you can get mail there)
7
Copy of baby's flight ticket home with the baby's name on it (for each baby)

Step 2: We'll help you get passport-compliant photos
Our team will check and prepare your baby's photos to meet passport requirements. Here's how:

Take a few photos of your baby with your phone or camera - show the full head and upper chest.

Make sure there are no shadows on your baby's face and use a white background.

Baby's eyes should be open if possible, but closed eyes are OK too.

Send the pictures to passport@surroplans.com CCing your SurroPlans advisor. Our team will check the pictures.
Once ready, we will email your passport photos back to you and you will be able to have them printed:
At your local "Walgreens"
Use the location finder here and ensure the "Passport Photo" is selected.
At your local "CVS"
Use the location finder here and ensure the "Passport Photo" is selected.
Step 3: Our passport team takes over
After we get and check your documents, our team will:

Fill out the DS-11 form for you and email it to you.
Please, DO NOT sign the DS-11. You will sign it at the passport agency during your appointment.

Send you a list of other items to prepare and print.
Step 4: What to bring to your passport agency appointment
Payment for the Passport fee.
Bring 195 USD to pay at the passport agency. You can pay with credit card. If you want to pay with cash, please tell us first and bring exactly 195 USD.
Baby passport photos (printed)
Baby's original birth certificate (no apostille needed - you will get it back with the passport)
1 photocopy of baby's birth certificate
Each parent's passport (originals)
Photocopies of each parent's passport (front page and back page)
DS-11 form (print single-sided, in color - DO NOT sign it - see instructions above)
Baby's flight ticket or reservation (must show flight number and baby's name).
Important: This list may change. We will tell you the exact documents needed when we schedule your appointment.
Step 5: At the passport agency - what to expect
Important: Each parent and the baby must come to the appointment in person.
SurroPlans team will:

Book your appointment at the passport agency

Send you the confirmation with the date and time.
You will:

Go to the passport agency for your appointment (be a little early to account for any delays)

Give your documents to the passport agency staff
3
Sign the DS-11 form at the appointment (do not sign it before)
4
Get your baby's passport - usually same day (pick up in afternoon) or  within 48 hours. We book appointments 3-4 days before your flight so you can leave with the passport.
Important: Each parent and the baby must come to the appointment in person.
