How to get your baby’s passport: Passport bureau appointment
Follow these simple steps to smoothly apply for your baby’s first passport.
Passport journey steps
The first information needed via email
Please, prepare and send us these documents and information:

Birth certificate of the baby - a scanned copy or a photo (for each baby)

Baby's eye color, hair color, height (for each baby)
3
Copy of passports — a scan or photo of the page with personal details (photo and signature), and the passport cover (for each parent)

Your home country permanent address

The email addresses you would like to use (if the ones copied to this email are not the right ones)

Your temporary address in the US (this is where the passport might be mailed so please make sure you have access to the mailbox)

Copy of the baby’s flight itinerary (or ticket) back home issued with the baby/babies name on it (for each baby)

How to make baby's passport picture
Once you have shared the information above (the most urgent information), we will need to get passport- approved baby pictures (for each baby)

Please take a few pictures/headshots of your baby with your smartphone/camera with all head and half chest.

Make sure there are no shadows on your baby or toddler's face.

It is better if baby's eyes are open but it is OK if they are not.

Send the pictures to passport@surroplans.com CCing your SurroPlans advisor. Our team will check the pictures and remove background.

Once ready, we will email them back to you and you will be able to have them printed:
At your local Walgreens
Use the location finder here and ensure the "Passport Photo" is selected.

What will happen next
Once we get your first email with the requested details, we will:

Fill out the DS-11 form for you and email it to you
Please, DO NOT sign the DS-11. You will have to sign it during the Passport bureau appointment.

Payment for the Passport fee.
Bring 195 USD that you will pay directly at Passport bureau (Credit Card will be accepted. If you want to pay cash, please check before with our Passport Service and remember to bring the right exact amount).
Approved and printed baby passport pictures
1 original copy of the baby birth certificate (Apostilled not necessary - it will be returned along with the passport)
A copy of the baby birth certificate
Parent's passports
1 copy of each parent's passport, front and back
DS-11 form (printed 1 sided, in color, NOT signed - see above)
Baby's itinerary flight ticket (The document needs to show the flight number and ticket or reservation)
What to expect at your ID validation appointment
Please, remember both parents and the baby need to be physically present at the Passport Bureau appointment.

Come to the bureau at the appointed time

Follow the Passport bureau procedure for submitting documents from the list of additional items to prepare and print

Get your passport by coming back to the bureau in the afternoon, or within 48 hours (we will do our best to schedule an appointment within 3/4 days of your departure to avoid any shipping possibilities).
